Here at Hazyland Old English Sheepdogs we are committed to producing healthy, happy and well socialised puppies.

We breed to a high standard and expect any prospective user of our stud services to do the same.

With this in mind we have designed the following form in order for us to make an initial assessment of the proposed mating.

Please do not leave it to the last minute to contact us as we would prefer to meet you and your bitch in person well before any planned mating.

Were there any problems with previous whelpings. If so, please provide details eg. not whelped before, no problems, caesarian section, stillborn pups etc
Has your bitch been hip scored?
If yes, what was the score? If awaiting result please state date hips were x-rayed
Do you have the original documentation of the hip score results?
Has your bitch been eye tested for Hereditary Cataracts?
If already eye tested please provide details of the reults and date/s tested
Do you have the original documentation of the eye test results?
Has your bitch been DNA tested for PCD?
If your bitch has been DNA tested for PCD what was the result?
Can you provide documentary evidence of the PCD result?
Are you willing to e-mail me a copy of your bitch's pedigree?
Please explain why you want to mate your bitch
